Annual Health and Medical records

Annual Health and Medical records need to be completed each year.  These forms are required for all camping although generally it associated with preparations for summer camp as it is required for summer camp also. 

The troop is required to keep a medical form no older than 1 year on file year round.

Generally, you will be contacted if and when the troop needs updated forms although it is your responsibility to keep your forms current.  If you have a question about dates or forms, please contact us at the following email:

The medical forms can change from year to year so in order that we get the most current Annual Health and Medical forms, check on the official Scouting web site. 
  1. If you are NOT going to summer camp, click Download in the section "All Scouting Events".  This will give you Parts A & B.
  2. If you are going to Summer Camp, choose the Download button under the section "Are You going to camp".  This will give you Parts A, B, and C

Forms are due 2 Jun 2018.  Parts A and B are filled out by the parents and part C must be filled out by your doctor.  If your scout"s well visit cannot be scheduled till after June 1 this is not a problem.  Please attach a note when you submit parts A and B letting me know this and submit part C as soon as possible.

Just a few reminders about medical forms
  • Scouts and parents must sign part A
  • On part A you may list Will Purcell, (732) 207-0896 as a designated adult to take your scout to and from events
  • On part B  Unit Leader Will Purcell    Council Name Monmouth Council  Unit No 122
  • On part B you must fill in your Insurance Policy Comp and the Policy No.
  • A copy of the front and back of your insurance card is required.  Even if it is the same as last year.
  • On Part B the Immunization section must be filled out or a copy of your scouts immunization record must be attached
If your scout in not attending summer camp I ask that you submit new parts A and B.  Medical records need to be kept up to date in order for scouts to attend weekend campouts.
