Medical Forms for Summer Camp

Dear  Parents and Leaders,

As you know the troop will be attending summer camp July 12th to July 18th.  It is time to start getting the medical forms necessary for camp in order.  Please visit the  troop website or the Boy Scouts of America website and search medical forms and click on the link “Are you going to camp” or use the link below

Parts A and B are to be filled out by the parents and part C by your physician.  Please turn in the completed forms no later than  May 21st .  Time is needed to review each form and allow time for corrections.

I know many insurance companies will only cover well visits on a “year to date” basis.  If you son’s well visit cannot be scheduled till after May 21st this is not a problem.  Hand in parts A and B, with a noted attached letting me know when the well visit is, and just turn in part C after the visit.  Please note scouts cannot attend summer camp without all three parts completed. 

If you scout will not be attending summer camp I ask that you fill out and turn in parts A and B.  These parts are necessary for weekend campouts.   Summer camp is also the time of year that the troop gets medical forms in order for the following scout year.

For parents of new scouts, if you have already submitted parts A and B, please submit new forms.  This will bring your scout upto date for the new scouting year.

Missing Tent

Everyone,
James Booth lent out his tent to someone on the Gettysburg Campout.  Please let us know if you have the tent of may know of its whereabouts.


Troop Car Wash

The annual Troop 122 Car Wash will be held at:

Veterans of Foreign Wars (VFW) Post 2179
Route 36 
Port Monmouth     
Saturday, May 9, 2015
9:00 AM – Noon.

Rain date: Sunday May 17, same time. 
            
Participation at the car wash is mandatory.

All Scouts must also sell five $5.00 car wash tickets in advance for a total of $25.00.  Tickets will be given out at the troop meeting on 16 April.  Sell as many tickets as you can - all proceeds go directly to the troop to help cover expenses andreduce dues.  Extra tickets will be available at all the meetings until the car wash.
 
Each scout should bring 4 old towels and any car wash/cleaning supplies (brushes, nozzles, etc) to the wash.
