Here are some forms and resources that parents may find helpful:

Medical

Medical Forms parts A, B, and C - These are required annually for summer camp and some other activities.  Parts A and B can be filled out by parents.  Part C requires a physician.

Troop Rules

Troop 122 Rules and Regulations - Rules and regulations for Troop 122.  Any questions about the running of the troop, check here first.  Still question, see Mr. Levine.

Troop Dues - The payment slip file is located below to cover all fees for the year for registration and monthly dues for camping permits.

Helping the Troop

Merit Badge Counselor Form - This is a new application (first time to register as a merit badge counselor). Attach this form to the BSA Adult Application, indicating position code 42

BSA Adult Application - This application must be filled out and submitted to the troop for any adult position to include merit badge counselor.  You can get a carbon copy form from the troop rather than filling out multiple copies of the online version.

Troop Resource Survey - Boy Scouting is for adults as well as boys.  The troop committee invites you to share your skills and interests so the best possible program can be developed for the Boy Scouts in this troop.

Driver and Vehicle Info - In order to obtain the tour permits necessary for our Troop to go camping, we need the following information for all vehicles in your household that may be used to transport Scouts.
